PICO question help for nursing students

We help you turn a clinical problem into a focused PICO or PICOT question: the patient population, intervention, comparison, outcome and, where needed, the time frame. You also get search terms and synonyms for databases such as CINAHL and PubMed, so the question leads straight into your evidence search and EBP paper.

What is a PICO question?

A focused clinical question with four parts: population (P), intervention (I), comparison (C) and outcome (O). Add a time frame (T) and it becomes PICOT. It's the first step of most EBP assignments.

How we build your PICO question

  1. Start from the clinical problem you've noticed.
  2. Make each element specific and measurable.
  3. Check it's answerable with research.
  4. Add search terms for CINAHL and PubMed.
  5. Explain each choice in a short rationale.

PICO question examples

  • In adults with type 2 diabetes (P), does nurse-led education (I), compared with usual care (C), lower HbA1c (O) within six months (T)?
  • In older inpatients (P), do bed alarms (I), compared with hourly rounding (C), reduce falls (O)?

Frequently asked questions

What is the difference between PICO and PICOT?

PICO stands for population, intervention, comparison and outcome. PICOT adds T, the time frame in which the outcome is measured, such as "within 30 days of discharge." Use PICOT when timing matters to the outcome or your instructor asks for it; otherwise PICO is usually enough.

What makes a good PICO question?

It names a specific population, one clear intervention, a realistic comparison such as usual care, and a measurable outcome, and research can answer it. For example: in adults after hip surgery (P), does early mobilization (I), compared with delayed mobilization (C), reduce pneumonia (O)?

What are examples of PICOT questions in nursing?

For example: In adult surgical patients (P), does chlorhexidine bathing (I) compared with soap and water (C) reduce surgical site infections (O) within 30 days (T)? Or: In older adults in long-term care (P), do hourly rounds (I), compared with usual care (C), reduce falls (O) over six months (T)?

Which question types can PICOT be used for?

PICOT works for intervention questions, and adapted versions fit other types: etiology (does an exposure increase risk), diagnosis (how accurate is a test), prognosis (what outcome is likely over time) and meaning (how do patients experience something), which often uses PICo for qualitative questions. We match the format to your question.

Do you include search terms for my PICO question?

Yes. For each PICO element we list keywords, synonyms and subject headings, such as CINAHL Headings and MeSH terms, combined with Boolean operators like AND and OR. That gives you a ready search string to run in CINAHL or PubMed and adapt as you refine your results.

Can you check a PICO question I've already written?

Yes. Send it with your topic and assignment, and we'll check that each element is specific, the comparison is realistic, the outcome is measurable and the question is answerable with current research. You get a revised version with a short explanation of each change.

Does a PICO question need a comparison?

Not always. Some questions have no clear alternative, such as how patients experience a new treatment, and the C can be left out or set as usual care. For intervention questions, though, a comparison makes the outcome meaningful, so most instructors expect one in a PICOT question.

What comes after the PICOT question?

The evidence search and appraisal. You search databases with your PICOT terms, appraise the studies you find, and synthesize them into an answer and a practice recommendation. We can help with each step, from the search strategy to the full EBP paper, ordered together or one at a time.
